SUPPORTED LIVING
Daystar Care provides supported living services across East Midlands to help and support individuals with a Learning Disability, Autism, Mental Health issues, and challenging behaviour.
These services are bespoke to the individuals needs and adaptations can be made to their environment to enable them to lead a safe and happy life. This is something Daystar Care does well for individuals within our care and further adaptations can be made when challenging or unpredictable situations arise.
We promote independence through the provision of intensive care and support for people whose characteristics and behaviours require understanding, robust risk management, structure and familiarity to enable engagement and integration into their communities by skilled staff. We support these people to achieve a good quality of life and well-being in sustainable homes leading to positive outcomes.
Before the commencement of our supported living services, we like to meet with the individual and their family. We meet the service user and their family to develop a strong understanding of their life story, identify their needs, and understand their aspirations. Once we have agreed that supported living is the appropriate service, we will work together to create a person-centred support plan.
- We will support individuals by:
- Helping them become independent with daily tasks
- Skills building to manage finances
- Developing healthy eating and lifestyle habit
- Supporting with medication and health appointments
- Managing correspondence and bills
- Building social skills
